I used to play this a ton as a kid in my weekly computer class in school, but I never got even close to beating it. I tried playing it a few times since but always got bored or died quickly. I eagerly went at it when I saw this post just now for nostalgia's sake. And I made it! For the first time ever! On my first attempt! It actually seemed pretty easy since I just followed the in-game recommendations on how much of what to purchase at the start from the shopkeeper and kept a close watch on my food. Hunted a lot since bullets are cheap, but still had to buy a ton of food later on. I should have bought more food up front. Almost ran out of food at the end, and did run out of money from buying more food...even with all the hunting.
Used common sense for the rivers, which I never understood how to do as a kid. Seemed sensible to do the fording option when it was less than 5 feet, the floating when it was ~10 feet, and whatever easier option there was if it was deeper than that. This worked out fine for me in all cases.
I always took the shortest option when choosing where to go, since traveling distance is the killer in this game. Wasn't worth going far out of my way to visit a fort when I had no more money to buy food anyway.
I never changed my pace away from the default, which was "steady" I think. I kept my rations at the middle option almost the whole time but changed it to the low option to barely be able to make it to Oregon with enough food at the end. I paid the toll for the easy path at the end since I kept ~$10 left figuring I might need it for something instead of using the last of it to buy more food.

Oh, and I almost forgot! Perhaps the most important thing that was the key to my success. Whenever my health was "poor" when arriving at a landmark, I would rest for 3 days for it to change to "fair" before proceeding. I happened to randomly rest at an early landmark and saw my health go from poor to fair so I knew that helped with that. If the 3 days didn't bring it up (which it usually did except a few times) then I would rest another 3 days until it did.
I assume the randomly dying occurs when your health is below fair.
